Value-Added Services are Effective September 1, 2025 - August 31, 2026.

Apply for Texas STAR (Medicaid)

24-Hour Nurse Hotline

24 hours Nurse Help Line for all Members to help them decide where to go to get care. Nurses also provide health education, follow-up, and network referrals.

Extra Vision Services

Members may opt-out of standard eyewear benefit and use $120 towards purchase of non-standard glasses or contacts, including contact fitting fee, every 24 months (STAR).

Sports and School Physicals

One sports league or school physical each year for Members aged 4 through 19.

Help for Members with Asthma

Yearly Members enrolled in our Asthma Care Management Program get:

  • Asthma educational materials.
  • One allergy-free pillowcase and mattress cover.

Based on prior receipt.

 

Extra Help for Pregnant Women

    • $25 gift card for completing prenatal checkup.
    • $25 gift card for timely postpartum checkup 21-84 days post-delivery.
    • App for pregnancy tracking, education, and planning.
    • A set of Community’s Precious Care Collection which include: NOTE: For pregnant women who download the ProgenyHealth* mobile app, enroll and complete an assessment, they receive the following.
      • Nursing pads.
      • One diaper bag.
      • One baby grooming kit.
      • For our pregnant women after using the Progeny app.

Home Visits

Up to 8 hours of respite services for post-partum care after completing a timely postpartum checkup within 21-84 days after giving birth.

Health and Wellness Services

$100 for annual BakerRipley membership;
Skill-building, food/nutrition, health promotion, wellness, exercise/social engagement activities.

Self-Management Wellness toolbox.

Healthy Play and Exercise Programs

  • $40/year for school sports expenses; Support for students up to grade 12. Play, learn, and earn.

  • $50 gift card each year for members up to grade 12 who participates in a youth sports league (apart from extra-curricular, school sponsored activities).

  • Age 6 years through 17 years who live in the Harris Service area may join a participating location of the Boys and Girls Club in the Greater Houston area for free.

Gift Card Program

  • $10 gift card for each of up to six well-child checkups.
  • $25 bonus gift card for completing all six checkups, total of up to $85.
  • 2 $25 gift cards for receiving 2 flu shots.

Extra Dental Services for Adults (age 21 and older) and Pregnant Women

Two routine dental exams and, one dental kit yearly with teeth cleaning, 1 x-ray annually, non-surgical extractions and emergency exams for 21 and older and pregnant women.

Transportation Services

  • Rides to WIC & food bank/food pantry appointments.
  • Members permitted to have additional riders as long as they are able to be safely transported in designated vehicle.

Disease Management

  • Diabetic accessory travel bag.
  • Daily sugar recording book/log.

Inpatient Follow-up Incentive Program

Members can receive: $40 gift card for mental health follow-up after seven days of inpatient visit.
