COVID-19 Cares Center In-Person Assistance Notice

For your health and ours, we are not offering walk-in assistance at this time. Please call 713.295.6704 to schedule an appointment or discuss other options for assistance. We will confirm your appointment and give you a phone number to text when you arrive. Save this phone number.

On the day of your appointment:

If you have any of these symptoms, please reschedule.

  • Cough
  • Shortness of breath or difficulty breathing
  • Fever
  • Chills
  • Repeated shaking with chills
  • Muscle pain
  • Headache
  • Sore throat
  • New loss of taste or smell
  • Fatigue
  • Nausea
  • Vomiting
  • Diarrhea
  • Abdominal pain
  • Runny nose

When you come for your appointment:

  1. Come by yourself, if possible. Limit 2 people.
  2. Text the phone number you received when you scheduled your appointment to let our Representative know you are here.
  3. Wait outside until our Representative texts you back before entering the building.
  4. Wear your face covering over your mouth and nose while you are in the building. If you remove it, we will not be able to assist you.

We will take your temperature and ask you a few COVID-19 questions when you come in. If your temperature is above 99.9 or your answers lead us to believe you may have COVID-19, we will not be able to assist you.

WHAT TO BRING IF YOU ARE APPLYING FOR COVERAGE:

  • Name and date of birth
  • Social Security number and/or immigration documents, number and type (if not a citizen)
  • Current household income (including tax adjustments such as student loan interest)
  • Employer name, telephone number, and address
  • Current health insurance information (insurance company name and policy number)
