24-Hour Nurse Hotline
Members can call a 24/7 Nurse Help Line to decide where to get care. Nurses also give health tips, check in after visits, and help members find doctors in the network.
Extra Vision Services
Members ages 0 through 99 can choose $120 allowance toward upgrades from standard eyewear once every 24 months for frames, lenses, contacts or fitting fees.
Sports and School Physicals
Members ages 4 through 19 get one sports or school physical each year from an in‑network provider.
Help for Members with Asthma
Members enrolled in Community’s Asthma Care Management Program get:

Extra Help for Pregnant Women
Home Visits
Women ages 18 through 65 can get up to 8 hours of respite care after giving birth. They must have a postpartum checkup within 21 to 84 days to qualify.

Extra Help Getting a Ride (when state services are not available)
Members can get transportation to WIC, food bank/pantry appointments by calling MTM Health or utilizing the app; extra riders permitted if they can be safely transported
Disease Management
Upon request, diabetic members ages 0-65 can receive:
Inpatient Follow-up Incentive Program
Members ages 13 through 65 get a $40 gift card for a follow‑up visit within 7 days after a mental health hospital stay
Online Mental Health Resources
Members 18 through 65 can get access to a mental health online companionship tool
