How to Enroll

Interested in becoming a Member of Community Health Choice (HMO D-SNP) plan?

Before making an enrollment decision, it is important that you fully understand our benefits and rules. If you have any questions, you can call and speak to a Member Services representative toll-free at 833.276.8306 (TTY users should call 711).

You’re eligible if you qualify for Medicare Part A and B, live in our service area and qualify for Medicaid QMB, QMB+, or SLMB+.

When you are ready to enroll, you may either enroll online or print an application and enroll by mail.

Preventative Care

With a Community Health Choice (HMO D-SNP) plan, preventive health care is covered at no cost to you. With this knowledge you and your doctor can work together to develop an action plan and identify adjustments to risk factors you can control.

Community Health Choice Texas, Inc. is a (HMO D-SNP) plan with a Medicare contract. Enrollment in our plans depends on contract renewal.
