STAR+PLUS Value-Added Services

Value-Added Services are effective September 1, 2025 to August 31, 2026.

Extra Help with Getting a Ride

Additional riders allowed to accompany members on trips.
Rides to food bank/food pantry, grocery shopping, Community sponsored events & senior recreational events.

Disease Management

Diabetic shoe insoles for members with diabetes and enrolled in Community CM or DM program.

Dental Services 

Two routine dental exams per year up to $600 with teeth cleaning, x-rays, (once annually), non-surgical extractions and emergency exams (limited) for Members 18 and older.

Extra Vision Services

Members may opt-out of standard eyewear benefit and use $150 towards purchase of non-standard glasses or contacts, including contact fitting fee, every 24 months.

Discount Pharmacy/Over-the-Counter Benefits

Members receive $10 per month (up to $120 annually) for over-the-counter-medicines and other health related supplies not covered by Medicaid.

Help for Members with Asthma

Members with Asthma or COPD in Community’s Disease/Case Management program get:

  • Asthma educational materials.
  • One allergy-free mattress cover.
  • One allergy-free pillowcase.

Extra Help for Pregnant Women 

  • $25 gift card for completing prenatal checkup.
  • $25 gift card for timely postpartum checkup 21-84 days post delivery.
  • App for pregnancy tracking, education, and planning.

Home Visits 

  • Up to 24 hours respite services for STAR+PLUS non-waiver Members.
  • Home Visits Companionship – Up to 48 hours companionship visits for STAR+PLUS non-waiver members.
     

Health and Wellness Services

  • 10 Home delivered meals for STAR+PLUS non-waiver Members after getting out of the hospital.
  • Access to a nutritionist to assist with dietary education and weight loss.
  • Free pill organizer.
  • Free blanket for newly enrolled. STAR+PLUS nursing facility Member.
  • Free digital, large print clock for newly enrolled STAR+PLUS nursing facility Member.
  • Free pair of non-skid socks enrolled STAR+PLUS nursing facility Member.
  • Community will give members a free flashlight to help them stay safe.
  • Free lighted magnifying glass with Community logo.
  • Free photo frame with Community logo.

Healthy Play and Exercise Programs

  • Healthy Play and Exercise Adult Skill – $100 for annual BakerRipley membership; Skill-building, food/nutrition, health promotion, wellness, exercise/social engagement activities.

    Self-Management Wellness toolbox.

  • Members get a free exercise kit; it will come with:
    • One medium resistance band.
    • One heavy resistance band.
    • And a pedometer to help track their steps.

Gift Card Program

Get rewarded for your healthly behaviors:

  • $85 for biannual HbA1c tests.
  • $30 for yearly diabetic eye exams.
  • $30 for diabetes screening with antipsychotic use.
  • $30 for annual mammograms.

Online Mental Health Resources

  • Online telehealth resources.
  • Online companionship tool.
  • Online mental health services including, individual and group therapy, aftercare services and medication management.
