Discharge Follow Up Care

Discharge Follow Up Care

The Importance of Follow Up Care

Mental illness is more common than we think. Caring for yourself during a mental illness crisis is a sign of strength and empowerment. If you are struggling or worried about your mental health, please treat it as you would a physical health condition. See a mental health provider.

When a hospital stay is necessary, it is important to follow through on next steps to recovery. A good follow-up care plan will help you manage you mental health after discharge.
