Complex Case Management (CCM)

Do you have two or more of the following diseases?

• Cancer
• Spinal Cord Injury
• Transplant
• Multiple trauma
• HIVA/AIDS
• Traumatic Brain Injury
• Diabetes
• High Blood Pressure

These are not all the diseases. You may have another serious disease as well. This condition is called a co-morbid condition.

If the answer is yes, you can join the Complex Case Management program offered by Community.

One of the best things about joining is that your doctor will explain how your diseases interact with one another. Your doctor will make sure you get the right treatment.
