Papa Pals: Community Health Choice Helps Members Thrive with Strategic Companion Care Partnership

Community Health Choice, Inc. (“Community”) announces today its partnership with Papa Pals to provide eligible Members with companion care services and support for everyday needs, improving Members’ whole health with personal connections.

“As Americans report increased social isolation and experts identify loneliness as a contributor to poor health outcomes,” said Lisa Wright, Community’s President and CEO, “we are investing in interventions and programs that help the whole person. Medicare Members insured through Community’s Dual Special Needs Plan (D-SNP) receive assessments for social needs related to each person’s physical and psychosocial functions and activities.”

Community, an innovative leader in health plan design, continues its focus on helping Members improve health outcomes. Community’s Life Services department was an early investor in the social determinants of health (SDOH), a recognition of the impact food, housing, transportation, and other factors have on Members’ health.

“We’re a health plan with heart,” says Wright. “Our mission to open doors of health and well-being to our Members is strengthened with strategic partnerships that provide Community’s Members with vital human connections that can increase well-being and contribute to better overall health. We’re committed to helping our Medicare D-SNP members stay connected and have the support they need to live their best lives.”
