Community Health Choice, Inc. (“Community”) launches an innovative partnership with Pyx Health to leverage technology for timely, meaningful, and personalized health care.

Community, a growing regional health insurer headquartered in Texas, is implementing new programs and services for Medicare Members covered under its Dual Special Needs Plan (D-SNP).

Isolation is a growing concern for Americans, particularly among older adults eligible for both Medicare and Medicaid, and social isolation is associated with a 50% increased risk of dementia and other serious medical conditions, according to the U.S. Centers for Disease Control (CDC).

“The CDC notes that loneliness and social isolation in adults are serious public health risks,” said Wright. “A recent report from the National Academies of Sciences, Engineering, and Medicine (NASEM), says more than one-third of adults 45 and older experience loneliness, and nearly one-fourth of adults aged 65 and older are considered to be socially isolated. We care about our Members living their best lives, so Community is taking action to help Members reduce isolation and increase healthy connections.”
