Community D-SNP plans include benefits designed to support your health, independence, and overall well-being. Your benefits depend on your specific plan and eligibility.
* The benefit(s) mentioned are part of a special supplemental program for the chronically ill. Members with chronic heart failure, cardiovascular disorders, chronic lung disorders, and diabetes may be eligible for these additional benefits. Having a qualifying condition does not guarantee the benefit; other requirements may apply. Unused monthly allowance funds do not carry over to the next month.
New for 2027, eligible members can earn rewards for completing certain health and wellness activities. Members may earn up to $245 per year.

Community Health Choice, Inc. is a Medicare Advantage (HMO D-SNP) with a Medicare contract and a contract with the State Medicaid Program. Enrollment in the plan depends on the plan’s contract renewal with Medicare and a contract with the State Medicaid Program.
