The Interoperability and Patient Access Rule (CMS‑9115‑F) allows you to use third-party apps to view certain health plan information. These apps help you see your plan and health data in one place. Learn more.

If you connect an app to Community Health Choice (Community), the app may receive:

  • Claims information
  • Visit and encounter details
  • Medication and pharmacy data
  • Lab results and vital signs
  • Immunizations and procedures
  • Care team and provider information
  • Basic demographic details
  • Choose a trusted health app.
  • Sign in to Community’s member portal and approve sharing.
  • Community sends your data through a secure connection.
  • You can stop sharing anytime by removing the app’s access.

Important: Once your data is shared with an app, Community cannot control how the app uses or protects your information. Always read the app’s privacy and security policies before connecting.

Your Privacy, Security, and Legal Rights

Community follows strong privacy and security laws, including the Health Insurance Portability and Accountability Act (HIPAA). Third‑party apps may not follow the same protections, so always review an app’s privacy policy before sharing your information.

Most third-party apps must follow the Federal Trade Commission (FTC) Act. The FTC helps protect consumers from unfair or misleading actions, including when a company shares your information without your permission.
