Accessibility Statement

Community Health Choice is continually improving our website in order to serve all audiences, including individuals with disabilities. Toward this effort, we’ve added assistive elements in order to meet the regulations outlined in Section 508 of the Rehabilitation Act. (Note: This opens to an external website)

For alternate access to web information:

At this time, the majority of information on the publicly accessible Community site should be accessible via screen readers and accessibility tools with the exception of some materials such as pre-2014 newsletters and certain dynamic content.  This information can be requested by contacting us.

If you need to convert PDF documents, Adobe offers conversion tools at its Accessibility Resource Center. (Note: This opens to an external website)

Adobe Acrobat Reader

Most of the documents on the website are in HTML. These formats are generally accessible to people who use screen readers. We also have a large number of documents in Adobe Acrobat Portable Document Format (PDF).

In order to view, and print PDF documents, you’ll need the free Adobe Acrobat Reader software. If you don’t already have it, you can download a free reader here. (Note: This opens to an external website)

PDF format is used to preserve the content and layout of our hard copy publications and forms. Publications in PDF can be viewed and printed using Adobe Acrobat Reader. The downloadable Acrobat Reader software is available at no charge from Adobe.

Generally, people using screen reading devices are unable to read documents in PDF format, unless they have an accessibility plug-in installed on their system along with the Adobe Acrobat Reader. This plug-in is available at no charge from Adobe. Adobe also has online tools that will convert PDF files to HTML on request. To get the plug-in and latest news about Adobe’s accessibility tools and services, visit the Adobe Acrobat Reader site. (Note: This opens to an external website)
