Community DualCare Aligned (HMO D-SNP) is a “HIDE Medicare Plan.” A HIDE Medicare Plan, or Highly Integrated Dual Eligible Special Needs Plan, is designed to coordinate benefits from both Medicare and Medicaid. HIDE plans merge Medicare and Medicaid benefits under one managed care organization (MCO).
Austin, Brazoria, Fort Bend, Galveston, Harris, Matagorda, Montgomery, Waller, and Wharton counties
To be eligible for this plan, you must qualify for Medicare Parts A and B and Texas Medicaid QMB, QMB+, or SLMB
Your plan gives you added support for your health and daily needs.

**The Flex Card is a special supplemental benefit for members who qualify and have a chronic illness. Eligibility is not guaranteed. Other rules may apply. Funds do not roll over from month to month.

A Special Needs Plan is also called an SNP. It serves people with certain health or care needs. It can also serve people who have Medicare and Medicaid.
It means the same managed care organization manages your Medicare and Medicaid benefits.
Aligned care can make it easier to find care and manage your benefits. Your doctors and care services can work together. You can also get help with visits, care, and questions.
Make sure your Medicare and STAR+PLUS Medicaid coverage use the same managed care organization. Your STAR+PLUS plan will follow your D-SNP plan if you live in Harris County and are fully insured through Medicaid, Check that your doctors and drugs are covered. Review your plan and enrollment choices.
You may align care during the Medicare Annual Enrollment Period, October 15 through December 7. You may also use the Integrated Care Special Enrollment Period if you qualify. If you live in Harris County and are eligible for full Medicaid benefits, you are allowed to switch to an aligned HIDE D-SNP once each month.
