Guide to Healthcare Services

Your Primary Care Provider is an important part of your healthcare team.

Your doctor will:
  • Make sure your childĀ gets the care they need, likeĀ regular checkups
  • Treat your childĀ when they are sick
  • Track everything about your childā€™sĀ past and present healthcare. Make sure they have all of yourĀ childā€™s medical records. You need to sign a form giving permission.
  • Make referrals to other doctors, if needed
You can pick any doctor in the Community network as your Primary Care Provider. You should pick a doctor with an office location and office hours that are convenient for you. If you like the Primary Care Provider that you see now, you can continue to see them if they are listed in Communityā€™s Network directory. To find a doctor online, click on the ā€œFind a Doctorā€ box below. You can also call Member Services atĀ 1.888.760.2600, and we will find you at least three doctors in your area to choose from. Once you have picked a doctor call Member Services. We will assign that doctor to you. We will also mail you a Member ID card with your new doctor information.

Healthcare Services Options

Primary Care Provider (Your Doctor)

Your doctor provides the following care:
  • Wellness checkups
  • Immunizations, including flu shots
  • Sports and school physicals, if youĀ participate inĀ sports
  • Back-to-school checkups
  • Sick and non-urgent conditions
  • Referrals to specialist, if needed
  • More
Some doctors offer walk-ins, but some require appointments. Contact your doctor to learnĀ their appointment preference and expected waitĀ time. To find a doctor online, click on the “Find a Doctor” box above.

Urgent and After-Hours Care

Urgent care centers provide treatment when you have an injury or illness that requires immediate care, but it is not serious enough to go to the emergency room. You should go to an urgent care center, if your doctor is not available.

Urgent care centers provide care for theĀ following:

  • Earaches
  • Minor/common infections (e.g. strep throat)
  • Minor cuts
  • Sprains/strains
  • Minor broken bones
  • Minor burns

They are open after regular doctor office hours. Walk-ins are welcome, but wait times may vary.

Emergency Room

Emergency rooms provide immediate treatment of life-threatening conditions. If you have severe symptoms or believe your condition is life threatening,Ā you should go to the emergency room or call 9-1-1.

Emergency rooms are open 24/7; however, wait times may be longer because patients with life-threatening emergencies will be treated first.
