Community Benefits Program

Community Health Choice (Community) will fund $750,000 in grants through its Community Benefits Program this fall in support of our organization's mission to improve the health outcomes and well-being of underserved residents in Southeast Texas, including:

  • Increasing the percentage of children who receive timely well-child visits and immunizations
  • Decreasing the rate of low birth weight babies by increasing the percentage of women who receive timely pre-natal and post-partum care, as well as managing care for women between pregnancies
  • Reducing the percentage of adults and pediatric potentially preventable admissions and/or emergency department visits
  • Increasing the number of parenting programs that provide training on child development and positive parenting strategies
  • Reducing obesity through programs that incorporate healthy eating and active living
  • Increasing access to integrated behavioral and physical care providers and /or programs

Funding for projects will be evaluated against the following criteria:

  • The estimated number of underserved population impacted
  • The potential to demonstrate increased service capacity (geographic or otherwise), for network providers that work with the underserved population
  • The potential to improve the cost efficiency of care as well as access and quality
