Community Health Choice Programs


Care Management Program

Our Care Management Program helps you manage your health. We focus on asthma, weight management, diabetes, and high-risk pregnancy. For more information, call Member Services at 1.888.760.2600.

Take charge of your health! Take our Health Risk Assessment online to see if you have any potential health issues. We will review it and contact you if we see any potential issues. Share your results with your provider.

Value-Added Program (Harris Service Area only)

Community will pay 100% of the membership cost, once per year, to the Neighborhood Centers, Inc. (NCI) of your choice for Medicaid members in the Harris Service Area. The membership only covers health and wellness classes. Limitations may apply. For more information, please call Member Services at 1.888.760.2600.

Text4baby App

If you are pregnant, Text4baby app helps you stay healthy during the pregnancy. You can receive free reminder text messages for things like labor sign and symptoms, prenatal care appointments, urgent alerts, and much more. You can also set up text message for you baby's health care until baby's first birthday. Sign up now!

State Case Management Programs


Case Management for Children and Pregnant Women

Need help finding and getting services? You might be able to get a case manager to help you. Who can get a case manager?
Children, teens, young adults (birth through age 20), and pregnant women who get Medicaid and:

  • have health problems
  • are at a high risk for getting health problems

Women, Infants, and Children (WIC)

WIC is a nutrition program for women, infants and children. WIC helps pregnant women and new mothers learn more about food, breastfeeding, formulas, nutrition, and healthy eating. WIC may help by giving WIC vouchers for healthy foods. Call our Member Services toll-free at 1.888.760.2600 to find a WIC office near you.

Early Childhood Intervention (ECI)

ECI is a statewide program for families with children, birth to three, with disabilities and developmental delays. If you are worried about how your baby is growing and learning, ECI can help you. ECI supports families to help their children reach their potential through developmental services.

You can find an ECI provider near you by calling the Texas Health and Human Services Commission toll-free at 1.877.787.8999 (TDD 7-1-1) or visit here. If you go to an ECI provider, please remember to tell your child’s primary care provider about the ECI care your child receives so that your provider may ensure continued care.
