That’s what the Community Rewards program is all about. You can earn wellness rewards, including 10% savings on your monthly premiums, for taking part in simple, healthy activities.

After you enroll, it all starts with a quick and easy health questionnaire. You can complete your health questionnaire online in about 10 minutes.
Only one adult (18 or older) is required to take a health questionnaire to earn the premium savings for everyone on your policy.
The early bird gets the savings.
- Members who complete the health questionnaire by November 25, 2018, will see the 10% savings on their January 2019 bill and all future 2019 Community bills.
- Members who complete their health questionnaire between November 26 and December 25 will see the savings on their February bill and all future 2019 Community bills but will have to pay their full January premium.
- Members who complete their health questionnaire between December 26 and January 31 will see the savings on their March bill, plus all future Community 2019 bills, but will have to pay their full January and February premiums.
- Members who qualify for a Special Enrollment Period must complete their Health Questionnaire within 30 days of their first month’s payment posting and their Member ID number/Member ID Card is issued. They will see the 10% savings applied to their bill beginning the second month and on all future 2019 Community bills.
It takes five to seven business days to process your health questionnaire results and post your wellness credit. So hurry! The sooner you complete your health questionnaire, the faster your savings will be applied. Once your health questionnaire is complete, you will receive 10% savings on your monthly premiums for the rest of 2019 as long as you keep your Community policy in place.
