READY TO RE-ENROLL?

When you’re ready, scheduled your free personal benefits review and select your plan for 2019. Three business day after re-enrollment you can log into your My Member Account and click on the Community Rewards icon to take a 10-minute health questionnaire. That’s worth 10% savings on your 2019 monthly premiums. Ask your Member Services expert for more details.

We have many plans to choose from, including Deductible plans for those who prefer to pay for healthcare expenses as they go, and Copay plans that work well for those who prefer predictable, up-front costs. We also have a new HMO Silver Deductible 009 plan that offers the best of both worlds—it’s a great value with lower premiums, too.

Community has a very large Provider network. If you have a doctor you like, you won’t have to worry about making a change.

New ways to save with our new HMO Silver Deductible 009 plan vs. other Silver plans

  • The premiums are lower – in some cases, much lower
  • There’s no deductible for primary care, outpatient mental health, and urgent care services or for generic prescriptions
  • Maximum out-of-pocket (MOOP) costs are the same—or less

We give you free 24/7 access to Telehealth

Community offers you Telehealth services, so you can speak to a board-certified doctor using your phone, computer, or any mobile device anytime of the day or night— free of charge.

We offer wellness incentives, including 10% premium savings

If you create a My Member Account and complete a 10-question health questionnaire by 11/25, you receive 10% off your 2019 premiums as a wellness incentive.

  • If you complete the questionnaire by 11/25, you’ll see the 10% savings on your January 2019 bill.
  • If you complete the questionnaire between 11/26 and 12/25, the 10% savings will be applied to your February 2019 bill, and you will be responsible for paying your full January premium.
  • If you complete the questionnaire between 12/26 and 1/31, the 10% savings will be applied to your March 2019 bill, and you will be responsible for paying your full January and February premiums
