2020 Enrollment Instructions and Application

You may be eligible for Community Health Choice’s (HMO D-SNP) plan if you answer “yes” to the questions below:

October 1 to March 31, 8:00 am to 8:00 pm, 7 days a week and April 1 through September 30, Monday through Friday, 8:00 am to 8:00 pm.

On certain holidays your call will be handled by our automated phone system.

Step 1. Check your eligibility

Call Member Services toll free at 833.276.8306 or 713.295.5007 October 1 through March 31, 8:00 a.m. to 8:00 p.m. 7 days a week, and April 1 to September 30, Monday through Friday 8:00 a.m. to 8:00 p.m. to confirm your eligibility for the D-SNP plan and enrollment period.

Step 2. Complete an application

Call us to speak with a licensed sales agent to enroll. Please have your Medicaid information ready

Complete an application and send to:

Step 3. Confirmation of application

You will receive a confirmation that we have received your application. Then we will process your application, confirming your Medicaid and Medicare eligibility for Community Health Choice’s (HMO D-SNP) plan.

Step 4. Confirmation of enrollment

You will receive a confirmation of enrollment if you are found eligible for the Community Health Choice (HMO D-SNP) plan.
