Application Process
The first step to becoming a CHC Provider is to contact a Provider Relations Representative. The second step is to complete the "Texas Standardized Credentialing
Application" that can be found and downloaded from the Texas Department of Insurance website. You can also request that CHC send an application to you.
If you are already
a Provider with one of the following IPA's already contracted with CHC, then you can become
a Provider by requesting that the IPA do an addendum to the contract,
which will include CHC. You will not have to fill out a credentialing
application.
