| For your convenience, our online Provider Directory allows you to search for in-network Providers. You can also get a map and directions to one of the CHC Network Providers, and provide a print-out to your patient. |
| CHC provides Health Education Tools for your patients. You can order or download these materials, free of charge, to distribute to your patients. Flyers, posters, bookmarks and other materials providing health information for pregnant women and parents of young children are available for to you. You can view a picture of the materials and email your request for materials. |
