Community Health Choice, Inc. (CHC) is a state certified and federally qualified Health Maintenance Organization (HMO). Our goal is to make it easy for you to work with CHC.We are sensitive to the demands on your time and resources, and are dedicated to offering the support you need by streamlining our administrative procedures.
