CHC Mission Statement

It is the mission of Community Health Choice, Inc. (CHC) to improve access to and delivery of affordable, comprehensive, quality, customer-oriented health care to residents of Harris County and its environs. CHC is committed to the following: Promoting individual and family health by making available comprehensive healthcare services to meet the needs of the person served; Developing community-oriented primary care and preventive health programs which reduce the incidence of illness and injury and promote wellness; and Establishing linkages between community Providers and CHC to improve accessibility and quality while reducing the cost of service.   CHC is committed to fulfilling its mission and has developed the following specific goals: 1. CHC is committed to providing a Quality Health Care Delivery System for its Members by:   Enforcing Provider credentialing and re-credentialing standards Performing clinical outcome studies Providing education Committing to Member wellness and prevention programs Encouraging quality outcomes and preventive care Committing to ongoing, internal, quality improvement processes Expanding internal/external service delivery system     2. CHC is committed to promoting accessibility of the Health Care Delivery System by:   Enhancing Member education regarding utilization of CHC services and the CHC Provider Network Continuing expansion of the CHC Provider Network Maintaining state-of-the-art communication technologies Enhancing network and Member service availability     3. CHC is committed to providing managed care products that are affordable by:   Minimizing network administration costs Effectively using case management, quality management, and utilization review Enhancing our ability to offer Provider Network options (flexibility)
